What it looked like to me was that the follow through is getting jerked across your chest which could explain the left to right ball movement. Maybe pull your hands back a little more on your backswing?

Joey - A couple of things to check -- How are you initiating your downswing. Make sure your Arms are following the lead of your legs/weight shift. Also - how are your shoulders lined up? Seems like they might be open a bit to the target which can cause you to come across the ball on the downswing.

Hey Joey, thanks for uploading your swing. Nick's dead on you have a reverse pivot. What this means is that you weight is shifting incorrectly throughout your swing. Specifically, on the backswing your weight is not over your back leg, it looks quite centered or even on your front foot. On your follow-through your weight should shift to your front leg - in your case your leaning back. Give this drill a shot - The Two Step Rhythm Drill - http://www.sirshanksalot.com/index.php?option=com_content&task=view&id=1094&Itemid=44 - there's a video there too to better show you how to perform it. Once you get your weight shift working for you, you should find your shots straightening out. Let us know how it goes! GDG

It looks like you've got a bit of a reverse pivot going on. That's probably why you look slightly unbalanced on your follow through
